Getting to know you

Developing a personalised plan begins with really getting to know you – your health and capabilities, your living arrangements and more. Most importantly we want to know what you want to achieve and how we can best work with you to achieve this.

Every client has a Care Coordinator. They are skilled professionals who oversee your care plan and know how to make complex things that affect your well-being run smoothly. It begins by undertaking an assessment with you to develop an individual plan so that you can remain living safely at home with a little help from us.

The RSL LifeCare Home Care team aims to have your GP and other health professionals informed and involved in your care and your loved ones too.

“One of my favourite pastimes is baking and cake decorating and I love to entertain my friends in my home. Having the beautiful carers from Home Care come to visit me a few times a week is a favourite part of my day. The co-ordinators in the office are very helpful and nothing is ever too much trouble. They’ve made such a difference to my life, helping me with shopping, cleaning and taking me on social outings. I would be lost without them.”
