Home Care Packages support you to live independently in your home and community.

Home Care Packages are tailored to suit you which means you can use your package to achieve your goals and live the lifestyle you’re looking for.

There are four levels of Home Care Package to meet different levels of needs, from Basic (Level 1) to High (Level 4).

Your package is individually tailored to your needs and priorities. This allows you, the customer, to choose the kind of assistance you want, determine how the funding in your package is spent, and change your choices if and when you choose.

What services can I choose for a Home Care Package?

The support you need and the things you enjoy are our top priority, so you’ll find a great variety of activities and services to include in your Home Care Package. There’s social outings, yoga and aqua aerobics, transport, cleaning and housekeeping, lawns and gardening, nursing and allied health, pet care and so much more for you to choose.

How is the cost of a Home Care Package covered?

The Australian Government subsidises the cost of many of the services offered in a HCP. Customers are asked to make a co-payment as well. The co-payment is made up of a basic daily fee and/or income-tested care fee. Therefore the level of individual contribution may vary.

Where do I start?

  1. Call our friendly team on 1300 853 146 or email us via the form and we will support you through the Home Care Package process.
  2. Register with My Aged Care: Your details can be registered by yourself or anyone you provide permission (your family or friends, a health professional or RSL LifeCare at Home) by contacting My Aged Care on 1800 200 422 or go to myagedcare.gov.au. Have an assessment by the Aged Care Assessment Team (ACAT)
  3. Receive Home Care Package approval and confirmation that you are on the waitlist
  4. Receive confirmation you are now assigned a Home Care Package
  5. Have an income assessment by the Department of Human Services (DHS)
  6. Choose RSL LifeCare at Home as your provider of choice
  7. Plan and develop your Home Care Package with RSL LifeCare at Home’s expert help
  8. Start living the life you choose

What if I need more help at home?

We are here to help. RSL LifeCare at Home provides a wide range of services under other programs that may benefit your needs. These include: Private Services, Commonwealth Home Support Program, Department of Veterans’ Affairs Community Nursing Program and Veterans’ Home Care.
