Your new role:
Due to significant growth of new RSL LifeCare clients requiring our support, we have a Care Coordinator position available in Canberra for our Home Care team.- You will be a key team member helping to deliver a variety of services to meet our client's unique circumstances and needs ranging from entry level through to complex health needs.
- With a large client base, you will oversee a portfolio of clients, develop and manage care plans, as well as create services and supports which are unique to individuals.
- Administratively, you will be responsible for a range of tasks such as drafting budgets, monitoring financial transactions, actioning changes in care, undertaking general marketing, as well as working with staff to deliver exceptional services.
Essential Criteria
- Demonstrated experienced in home care packaging (HCP), CHSP, DVA, case management, assessment, care planning and service coordination
- Current CPR Certificate
- Solid understanding of client centred, community-based care service delivery and associated quality standards
- Experience working with and adhering to budgets
- Ability to liaise with and promote our services to external providers
- Ability to create and maintain interpersonal relationships with staff
- Current drivers' license with own fully insured vehicle
- Proof of Covid-19 vaccination status

To succeed in this role, our expectations are that you will:

As the Clinical Care Manager, you will lead and manage the provision of appropriate clinical care to our clients living at home. You will provide direction, support and guidance to our team of Registered Nurses out in the field ensuring they are delivering a person-centred approach, recognising and promoting the client's unique goals and preferences.
Some aspects of your new role will see you:- Working with Education team colleagues to develop, implement and monitor consistent, best practice policies and procedures across all teams and locations
- Supporting Clinical and Care team members work to embed quality care and clinical governance systems and processes, aligned to the LifeCare Way
- Supporting teams actively participate in care and clinical governance systems in order to meet and maintain accreditation requirements
- Work with the Community Services Managers, Regional Manager and care teams to identify, mitigate and resolve actual or potential risk and ensure lessons learned are shared
- Role-model a culture and approach where clients are the focal point in decision making and the design of care delivery models
- Participate in and contribute to incident management and investigations as required
- Participate in an on-call roster for clinical escalation for evenings and weekends as required
- Demonstrate leadership through effective interaction with nursing and care teams

Responsibilities of this role would be, but not limited to:
Reporting to the Site Manager, you'll lead a committed kitchen team in delivering high-quality, nutritious, and resident-focused meals.- Oversee the day-to-day supervision of employees regarding all aspects of the current food operation
- Demonstrating strong leadership and creating and sustaining a high-performing team.
- Preparation, planning, and cooking of various meals, including breakfast, lunch, dinner, and snacks
- Complying with all infection control protocols and safe food handling procedures
- Ensuring all meals are prepared whilst keeping resident choices, allergies, and recommendations in mind
- Supporting WHS and infection control practices
- Work collaboratively with the care, clinical, and lifestyle teams
- Ensuring regular audits are being performed and recorded monthly
- Maintain food costs within budgetary guidelines
- Maintain ordering systems and supervise ordering for all sections in the main kitchen including textured and special diet sections
- Engage with residents on a regular basis ensuring their feedback is listened to and considered
Skills and Experience required:
- Certificate in Commercial Cookery or equivalent
- Experience managing kitchen operations and leading teams
- Knowledge of special dietary needs and aged care food standards
- Ability to plan and implement catering services, ensuring a holistic approach and having regard for the resident
- Creativity in menu planning and recipe management tools
- A team mindset, compassion, and a can-do attitude
- Aged care experience is desirable but not essential
- Demonstrated understanding of quality control in food preparation and delivery within an aged care setting
- Australian Working Rights

We are seeking a passionate and experienced Quality Coordinator to lead clinical governance and drive continuous improvement across our Residential Care facilities.
About the Role:
As a Quality Coordinator, you will work collaboratively within a multidisciplinary team to ensure the consistent delivery of safe, high-quality care. You will provide leadership in clinical risk management, regulatory compliance, and quality improvement initiatives. This role is pivotal in supporting Facility Managers, Clinical Care Managers, and staff to maintain compliance with legislation, organisational policies, and the Strengthened Aged Care Quality Standards.
Responsibilities include:- Overseeing quality audits and compliance with organizational policies.
- Leading continuous improvement initiatives to enhance resident outcomes.
- Supporting and delivering staff training, ensuring adherence to mandatory training requirements and orientation programs.
- Working within a multidisciplinary team to provide guidance, ensuring best practice care standards.
- Analyse performance data to identify trends and implement improvement strategies.
- Prepare accurate reports on quality outcomes and compliance.

About You:
We're looking for someone passionate about driving quality and training in aged care. You bring:- A Registered Nurse with valid AHPRA registration and a minimum of five years' RN experience, preferably with background in quality management or aged care.
- Strategic and clinical planning skills, with an understanding of organizational behaviour.
- Expertise in change management and cultural change processes, with strong critical thinking.
- Ability to identify improvement areas and implement initiatives for better compliance and client satisfaction.
- Demonstrated knowledge of Aged Care Quality Standards, NSQHS, and regulatory compliance requirements.
- Strong communication skills to deliver impactful, persuasive messages.
- Excellent relationship-building and interpersonal skills for sustainable outcomes.
- Collaborative within multidisciplinary teams.
- Self-motivated with the ability to work unsupervised to meet deadlines
- Adaptable to changing demands and priorities.
- Mandatory clearances before starting Police Check or NDIS Worker Screening Check
