Post-Hospital Care is one of the key Home Care services we can provide for you. Our Support Workers are highly trained to facilitate your transition from the hospital to your home, providing support throughout your recovery process. Our primary goal is to ensure your smooth and comfortable return home, allowing you to rest, recuperate, and regain your health.​

Following a hospital stay, most of our clients undergo a re-assessment conducted by our Care Coordinator as part of our restorative care approach. Your recover is overseen by the Care Manager, who takes into consideration your input regarding your physical abilities post-illness. Over the course of several weeks, we work closely with you to enhance your physical strength and boost your self-confidence. ​

The post-hospital care services we can provide for you include:

  • Liaising with hospital discharge planners in organising your transition home
  • Understanding your illness and liaising with the hospital staff regarding your specific recovery plan
  • Medication management
  • Assisting with all your domestic and personal care needs
  • Assisting with follow up appointments and liaising with your GP and other allied health providers
  • Giving you the positive encouragement you need and being there for you
